Add me as a facebook friend, and then as a developer on your application settings page.
Also send me the direct link to your facebook application settings page.
I will then be able to look at your settings.
It seems to me, though, that your host server is simply too slow. The facebook connection might be timing out.
To be honest, I doubt that your application will be usable on that server. I would suggest that you move your site to a faster host.

sory to bothering u, i know the problem.
i think its from my host with my facebook.
but i cant change my hosting cause im in indonesia, and my customer from indonesia.
i already sent my ticket to my hosting bout this...
